Baseball
registration is for ages 4-15.
Player
must turn 4 by April 30, 2010 or cannot turn
16 before April 30, 2010.

A copy of
your child's birth certificate is required
if you have not played at OPAA before. You
will also need to know your child's shirt
and pants size. Samples will be on hand at
registration.

How to Register
for Baseball at the Park
- Arrive at the field house with a
copy of your child's birth certificate if it is your
first time.
- Fill out Registration Form and
Medical Release Form (click Registration button to
download). Some registration periods will have a Notary
present to notarize forms. If no Notary is present, you
will need to keep this form, get it notarized and hand
it to your Coach at the first practice.
- Read the Parent Behavior Form.
- Make registration payment by
cash, credit card, or check.
- Registration forms may be printed
from the links below and completed before arriving.
